A financial agreement is required to be signed by all parents prior to your child receiving their O'Charley's Breakfast Fundraiser tickets.

By submitting the information below you are e-signing and you acknowledge that you are responsible for the cost of your child's tickets in the event tickets are lost or misplaced.

The following fees apply to lost or misplaced tickets:
• $40 for Varsity and JV Players – 2 Tickets, $20 per ticket
• $20 for Practice Players – 1 Ticket, $20 per ticket

You further acknowledge that you will return any unsold tickets for the O’Charleys breakfast.  Money and any unsold tickets will be returned to a booster board member or placed in the coach’s black cash box on or before practice on August 31st.
