With registration of my child in the CM High School Soccer Boosters Camp, I hereby, for my child and me, waive and release any claim we may have for damages against CM Boys and Girls Soccer Boosters and their staff for any and all injuries that my child may incur as a result of participating in the above‐referenced program. I also certify that my child has been released by a licensed medical professional within the past year and is in good health.  CM Soccer High School Boosters also reserves the right to make decisions related to refunds for cancellations due to but not limited to an Act of God or inclement weather.
