This form is being used to close out our season. You have until June 15, 2020 to respond. There are three options to select from.
1) Donate your registration fees to the league.
2) $125 credit per player towards next season (up to $375 per family).
3) $75 refund per player (up to $225 per family) in the form of a check.
If we don't hear from you by June 15th, we will consider your registration fee(s) a donation towards lost funds from this season.
