WAIVER for campers UNDER 18

WAIVER for campers 18 and OLDER

Please follow the instructions in completing, saving and sending the form:
 

  • Download waiver and save it to your desktop in the following format:
    Last Name, First Name, Camp (i.e. Residential, Team, Day, ID)

  • Read and fill out the form in it's entirety.

  • Email completed form to dasacamp@gmail.com

We must have this form in hand by registration in order for your camper to participate in camp.

Dear Campers and Parents:

 

Furman University has released their COVID protocols for all camps being held on campus this summer. All campers, parents, and staff must comply with each of the following policies.

All of these protocols are to ensure the safety of our campers and staff. We want to have a successful and fun camp this summer and these protocols will make it happen.

 

  • All full day campers and staff must provide one of the following:

  1. NEGATIVE PCR (nasal or saliva) COVID test taken within 5 days of the start of camp. Rapid antigen COVID tests will not be accepted. There are many FREE testing sites available with a 24-hour turnaround.

  2. PROOF OF VACCINATION for eligible participants

  • This proof may be emailed to dasacamp@gmail.com.

  • All campers and staff must complete and submit a COVID waiver form prior to registration day. This form may be downloaded from https://www.soccercamp.com/covid-waiver and the completed form should be emailed to dasacamp@gmail.com prior to registration.

  • All campers and staff are recommended to wear masks when they are indoors with the exception of eating or drinking.

  • Daily screenings will be conducted by our staff/trainers for all campers and staff.

  • If anyone is feeling ill and showing symptoms related to COVID, they will be immediately isolated, and a guardian will be contacted to pick up the child.

  • We recommend that all parents, campers, and anyone else attending registration to please wear a mask throughout the duration of check-in. Social distancing will not always be available so we like to keep everyone safe.
