COVID SCREEN: IF YES TO ANY OF THESE YOU MUST STAY HOME
1. Temperature ABOVE 100.4F today?
2. Do you have any of the following symptoms?
• Fever or chills
• Diarrhea
• Cough or sore throat
• Shortness of breath or difficulty breathing
• Fatigue
• Atypical muscle pain or body aches
• Headache
• New loss of taste or smell
• Congestion or runny nose
• Nausea or vomiting
3. Have you had COVID-19 within the last 14 days, or tested within the last 14 days due to exposure, contact with infected person and/or symptoms? (Mandatory testing/screening and/or negative precautionary test w/ no symptoms can be considered NO).
4. Within the last 14 days, have you been exposed to, or come into contact with anyone:
(a) who has COVID-19
(b) who is/was being tested for COVID-19 (excludes mandatory screening where no exposure, symptoms or sickness is involved)
(c) who had symptoms consistent with COVID-19 (d) who was exposed to someone with COVID-19
• where "contact" or "exposed to" is CSC defined as "being within 6 feet of an COVID-19 positive person for 15+ min duration, or having actual physical body contact". https://www.cdc.gov/coronavirus/2019-ncov/php/public-health-recommendations.html
•• you can answer NO to #5 if you are fully asymptomatic AND either one of these apply:
a/ within the last 3 months you tested positive for COVID-19 (viral test) & clinically recovered from COVID-19.
b/ within the last 3 months you are 2-weeks past receipt of full vaccine (i.e., second dose in a 2-dose series, or one dose of a single-dose vaccine). NJ Health Quarantine LINK HERE
